By submitting this application, I acknowledge that I am the parent or legal guardian of the applicant and understand the purpose and expectations of The W.Y.S.E. Experience Summer Leadership Intensive.
I understand that this program is designed to provide leadership development, mentorship, character education, personal growth, and life skills through classroom instruction, group discussions, team-building activities, guest speakers, and other supervised program experiences.
I acknowledge and agree to the following:
* My child is expected to demonstrate respectful behavior, actively participate, and follow all program rules and expectations.
* Repeated disruptive, unsafe, disrespectful, or inappropriate behavior may result in dismissal from the program when deemed necessary.
* I understand that every reasonable effort will be made to provide a safe and supportive environment; however, participation in any youth program involves inherent risks that cannot be completely eliminated.
* I authorize Water Your Seedz Everyday, LLC, its employees, volunteers, mentors, and representatives to seek appropriate emergency medical care for my child if I cannot be reached in a timely manner. I understand that I am responsible for any medical expenses incurred.
* I release and hold harmless Water Your Seedz Everyday, LLC, The W.Y.S.E. Experience, its officers, employees, volunteers, mentors, sponsors, and partnering organizations from liability for injuries, losses, or damages arising from participation in the program, except where caused by gross negligence or willful misconduct.
* I understand that photographs, video recordings, and audio recordings may be taken during program activities for educational, promotional, and marketing purposes, including use on the organization’s website, printed materials, presentations, and social media platforms. I grant permission for my child’s likeness to be used for these purposes without additional compensation unless I indicate otherwise in writing before the program begins.
* I understand that submission of an application does not guarantee acceptance into the program. Applications are reviewed based on program eligibility, available space, and the overall needs of the cohort.
* I understand that the required commitment fee reserves my child’s place in the program and is subject to the refund policy provided by Water Your Seedz Everyday, LLC.
By checking the acknowledgment box and submitting my application, I certify that I have read this waiver in its entirety, understand its contents, and voluntarily agree to these terms on behalf of myself and my child.
